You are responsible for the accuracy of the information you enter and are encouraged to periodically review your information. Your are responsible for all of your login information including to whom you give your user name and password.

The Content includes both Content owned or controlled by OMR and Content owned or controlled by third parties and licensed to OMR, but does not include Subscriber’s personal medical information. Except as set forth below, direct or indirect reproduction of the Content, in whole or in part, by any means, is prohibited without the express written consent of OMR.

SITE ACCESS AND LICENSES

OMR grants a limited license to each Subscriber to make personal use only of the Web site and the associated services in accordance with these terms and conditions of use. This license expressly excludes, without limitation, any reproduction, duplication, sale, resale or other commercial use of the Web site and the associated services (other than the reproduction or duplication of Subscriber’s records by them or authenticated medical personnel), making any derivative of the Web site or the associated services, the collection and use of user e-mail addresses or other user information, including, without limitation, health information or any data extraction or data mining whatsoever.

SECURITY

This Web site uses encryption software. While we use state-of-the-art security, no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information sent to us. When using our Web site information will be transmitted over a medium that is beyond the control of OMR and its contractors. Once the information is received by us, your medical information will be treated as confidential and given the same protection that all other OMR medical information is given.

CREDIT TRANSACTIONS

OMR’s annual fee of $24.95 is payable by using PayPal or Google Checkout, which reserves the right to accept payment from your credit card. You will be billed each year for the next year’s subscription fee of $24.95 sixty (60) days in advance of renewal date. You will be required to pay your annual fee through PayPal or Google Pay prior to the renewal date. OMR retains no credit card information.

SERVICE

OnlineMedicalRegistries.com (OMR) provides you with a private, protected file and permits medical personnel who we have verified are in the ER of a hospital, to view and print your advance directive, insurance and “Care Community” contact information and your Google Health records. Although OMR stores your personal identifying information, which is necessary in order for a hospital to accurately find your records, OMR does not import or save any portion of your Google Health record. Subscribers can also upload a scanned advance directive to OMR. OMR’s e-mail based notification system automatically contacts your “Care Community” when your information is accessed by emergency medical personnel.

BY ENGAGING OUR SERVICES, YOU ACKNOWLEDGE AND AGREE TO SOLELY ASSUME THE RISK OF THESE LIMITATIONS. YOU FURTHER UNDERSTAND THAT NO WARRANTY OR GUARANTEE HAS BEEN MADE TO YOU CONCERNING ANY PARTICULAR RESULT OR CURE OF YOUR CONDITION.
